Opportunity to deliver an Enhance project in Yeadon area
The Enhance programme which was launched in May 2022 is about to move into its third year.
Whilst the programme has had great success in some areas with Leeds Community Healthcare (LCH) Neighbourhood Teams (NTs) and Enhance delivery partners (DPs) developing and strengthening good working relationships, the Yeadon area has been slow to establish.
As we know that this NT is one of the busiest and under the greatest pressures and that the community within the Otley and Yeadon ward with a population on high levels of older people and those living with frailty, we aim to develop better engagement in this area and larger numbers of referrals. As the focus of the evaluation for year 3 is to provide evidence of time saved for LCH teams, it has been agreed to develop a fresh approach for the Yeadon NT area.
This area has a large geographical cover that incorporates Yeadon, Rawdon, Otley and Bramhope and we are inviting applications from third sector organisations who work to support older people and who know this area and the community within it to make an application for funding to deliver an Enhance project.
